Dear Parents/Guardians,

Enclosed you will find the registration packet for the 2007-2008 school year. Registration will be as follows:

• March 5th   Families already enrolled in the school
• March 17th St. Thomas Parish families not currently registered and other community members

            To complete registration, please return the following forms:

• Registration and Tuition agreement – one per family
• Information and Emergency contact form – one per student
• Field Trip permission slip – one per student
• General photo release – one per family
• Class session selection sheet
• Birth Certificate & Baptismal Certificate (if we already have this on file for your student, no need to include it again)
• Immunization information (will be required by August 15th)

Please contact us if you would like information about scholarship possibilities. We are looking forward to a wonderful second year at St. Thomas Academy!
